Instructions for completing your report:

• Note: Don't really need this: Type in submission code mcamentorreport
• Complete all areas pertinent to your Cadet including all contact dates and placement activities.  Placement activities include employment, education, military, volunteering.
• If no contacts were made a report is still needed with an explanation in comments/notes area.
• If the Cadet has moved from the area, has enlisted in military or gone off to school, updated information can be gathered from family members.
• If placement includes employment please verify with employer.
• Click on RPM Assistants and hit submit.
